In fact, the human bio-energetic field is measurable up to 15 feet into space (1).\u00a0This localized field of energy and information is\u00a0embedded within,\u00a0and intertwined with, a\u00a0universal matrix of energy and information. Every cell is sensitive to,\u00a0and dependent upon, the flow of energy.\u00a0\u00a0The advent of modern physics has opened the door to\u00a0the age of \u201cEnergy\u00a0Medicine,\u201d\u00a0one aspect of which\u00a0involves\u00a0the\u00a0Pulsed\u00a0Electromagnetic\u00a0Field\u00a0(PEMF).\u00a0Two decades ago one of my great teachers, Dr. Deepak Chopra, declared that one day we would heal with energy\u00a0\u2014\u00a0we would know the vibrational frequency, say, of a healthy liver and be able to apply that frequency to heal an unhealthy liver.\u00a0 PEMF\u00a0therapy is indeed a step in that direction.\u00a0What\u00a0is Pulsed Electromagnetic Field\u00a0(PEMF)\u00a0therapy?\u00a0PEMF therapy can be envisioned as a\u00a0type\u00a0of physical therapy employing therapeutic,\u00a0time-varying, low&#8211;energy\u00a0pulsed\u00a0electromagnetic fields. These\u00a0pulsed\u00a0electromagnetic fields\u00a0offer\u00a0specific\u00a0wave\u00a0signal shapes and are of extremely low frequencies, generally between 5 and\u00a0100\u00a0hertz.\u00a0The\u00a0impact of\u00a0both\u00a0the earth\u2019s natural\u00a0electromagnetic\u00a0field and\u00a0externally applied PEMFs influences many biological functions.\u00a0On the cellular level\u00a0the membranes of healthy cells have positive\u00a0and negative magnetic charges.\u00a0These charges are necessary for the exchange of\u00a0ions\u00a0carrying chemical elements in and out of the cells.\u00a0If cells deteriorate or\u00a0become\u00a0injured,\u00a0they can experience a\u00a0reduction in their\u00a0electromagnetic charge and thus a reduced ability to exchange ions.\u00a0PEMF therapy\u00a0can assist\u00a0in the restoration of the cellular\u00a0electromagnetic charge\u00a0enhancing cellular energy production.\u00a0Experiments have been constructed with many types of PEMF therapies, and each type of PEMF therapy involves many variables\u00a0\u2014\u00a0such as hertz frequency, waveform and magnitude,\u00a0and duration of treatment.\u00a0Each of these\u00a0parameters can\u00a0modify\u00a0specific biological responses and therapeutic effects.\u00a0PEMF therapy was first tested by NASA in the 1970s\u00a0to help alleviate astronaut muscle and bone loss,\u00a0and later\u00a0this therapy gained\u00a0approval from\u00a0the FDA for\u00a0varied\u00a0clinical treatments.\u00a0Although the potential of PEMF therapy was recognized decades ago, even today this therapeutic modality remains on the outer margins of Western medicine.\u00a0Much of the PEMF research has been conducted in China and Russia and\u00a0many of\u00a0these research\u00a0reports\u00a0are not available\u00a0in\u00a0English-language medical databases.\u00a0\u00a0PEMF and\u00a0bone\u00a0\u00a0The bone-building potential of\u00a0PEMFs\u00a0(Pulsed\u00a0Electromagnetic\u00a0Fields) has been a topic of interest to me since 1985,\u00a0when\u00a0I was exposed\u00a0to\u00a0the\u00a0work of\u00a0orthopedic surgeon,\u00a0Dr. Robert Becker.\u00a0His book (2),\u00a0The Body Electric,\u00a0introduced me and much of the world to the possibility that\u00a0certain\u00a0electrical fields\u00a0could stimulate bone regeneration.\u00a0Today, 35 years later,\u00a0one can find\u00a0more than\u00a05,000\u00a0scientific articles on PEMF and bone health, yet there are still many uncertainties and few human clinical trials.\u00a0In the 2000s\u00a0researchers\u00a0indeed verified that\u00a0PEMFs\u00a0could\u00a0stimulate\u00a0healing of selected\u00a0fractures (3).\u00a0Soon bone stem\u00a0cells became\u00a0a\u00a0focus of research and it was found that application of an alternating electric current to these mesenchymal cells could\u00a0encourage these stem cells to develop into bone-building osteoblasts and promote fracture healing (4).\u00a0Today\u00a0in 2021 the use of \u201celectrical stimulation\u201d to help resolve non-union fractures\u00a0where fracture healing is\u00a0impaired, and\u00a0to\u00a0heal\u00a0breaks of the upper limbs\u00a0is a commonplace medical practice (5,6,7).\u00a0Further, it is well accepted that PEMFs effectively relieve chronic bony\u00a0pain without side\u00a0effects (8).\u00a0&nbsp;Can\u00a0Pulsed\u00a0Electromagnetic\u00a0Fields (PEMFs) help prevent and treat osteoporosis?\u00a0Given that\u00a0PEMF\u00a0therapy\u00a0has been shown to speed\u00a0healing of recalcitrant non-union fractures,\u00a0the next\u00a0obvious\u00a0question\u00a0is, \u201cCan pulsed\u00a0electrical frequencies\u00a0help to build bone density?\u201d\u00a0The answer is likely \u201cyes.\u201d\u00a0However, there are many variables\u00a0that\u00a0impact PEMF treatment and too few human clinical trials have been conducted to allow for a definitive\u00a0bone-enhancing protocol.\u00a0As\u00a0we wind down 2021,\u00a0PEMF has been demonstrated to reduce pain and improve functional outcomes of patients with osteoporosis,\u00a0but few studies have been conducted\u00a0attempting to\u00a0enhance\u00a0bone density and strength\u00a0with PEMFs (9).\u00a0Successful animal\u00a0and test-tube cell\u00a0studies, however,\u00a0suggest\u00a0various\u00a0mechanisms by which PEMFs\u00a0could help prevent and treat osteoporosis. Among other actions, PEMFs activate\u00a0various intermediaries and hormonal pathways which\u00a0likely\u00a0end up enhancing\u00a0osteoblast\u00a0bone-building activity\u00a0and\u00a0limiting\u00a0osteoclastic\u00a0bone\u00a0breakdown.\u00a0Human\u00a0studies\u00a0using PEMFs\u00a0for\u00a0osteoporosis\u00a0Surprisingly, there\u00a0are\u00a0few human trials\u00a0on Pub Med\u00a0using PEMF\u00a0therapy\u00a0to prevent or treat\u00a0osteoporosis.\u00a0A\u00a0Chinese\u00a0study,\u00a0for example,\u00a0looked at the use of PEMFs\u00a0for\u00a0postmenopausal women with osteoporosis.\u00a0In this study a small group of women were treated with PEMFs\u00a0(at\u00a08 hertz, intensity of magnetism of 3.82\u00a0mT,\u00a0and 40 minutes\/day)\u00a0over 6 months.\u00a0Although this was\u00a0a\u00a0study of short duration,\u00a0results suggest that this\u00a0course of PEMF\u00a0therapy\u00a0was as effective as Fosamax (alendronate) in treating postmenopausal osteoporosis\u00a0over the 6\u00a0months.\u00a0Specifically, there was no significant difference in the mean percentage change of BMD in lumbar spine and left proximal femur between PEMF and alendronate groups from baseline to 24 weeks(10,11).\u00a0Another,\u00a0shorter&#8211;term study exposed\u00a020\u00a0patients to 100 hertz\u00a0PEMF\u00a0exposure\u00a0for 60\u00a0min\/day 3x a week over 3 months and found no significant increase in bone mineral density\u00a0from placebo.\u00a0However,\u00a0in this short time period\u00a0those treated with PEMFs\u00a0experienced an increase in bone formation markers. These markers returned to baseline 1 month after the end of treatment(12).\u00a0\u00a0A\u00a0small,\u00a0controlled trial tested if PEMF treatment could reduce\u00a0the bone\u00a0loss\u00a0that occurs with forearm disuse (as wearing a cast due to fracture,\u00a0or disuse due to\u00a0carpel\u00a0tunnel surgery).\u00a0PEMFs\u00a0of 15 hertz were\u00a0applied for 1, 2,\u00a0or 4 hours\u00a0per\u00a0day for 8 weeks.\u00a0Although there was a substantial variation in the bone loss associated with\u00a0disuse, the results of this study suggest that the PEMF waveform\u00a0for the\u00a0duration applied did not substantially reduce bone loss in the subjects(13).\u00a0\u00a0Hawaiian researchers, on the other hand, reported\u00a0that\u00a012 weeks of 72 hertz\u00a0PEMF\u00a0exposure\u00a0to the forearm\u00a0for 10 hours\u00a0per\u00a0day significantly increased\u00a0wrist\u00a0bone density in the immediate area of the PEMF\u00a0field exposure.\u00a0A similar but weaker response occurred in the opposite untreated arm, suggesting a \u201ccross-talk\u201d effect.\u00a0Researchers suggest this might have been due to weak general field effects or possible arm proximity during sleep.\u00a0This bone density gain was lost over the next 36 weeks(14).\u00a0\u00a0All in all, the handful of\u00a0available\u00a0clinical trials\u00a0looking at\u00a0the bone&#8211;building capacities of\u00a0PEMFs\u00a0report contradictory and controversial results. Multiple reasons can be listed for these varied and often contradictory results.\u00a0Even with\u00a0varied results,\u00a0I and\u00a0other\u00a0PEMF researchers\u00a0maintain optimism about\u00a0the bone-building potential of PEMF therapy (8).\u00a0\u00a0Some important variables\u00a0to consider when analyzing PEMF studies\u00a0include:\u00a0\u00a0Different clinical designs using varied frequency ranges from 8 to 100 hertz\u00a0Varied field range intensities\u00a0\u00a0Varied numbers of PEMF treatments\u00a0per day\u00a0\u00a0Variations in length of study duration\u00a0(on-going treatment may prove necessary)\u00a0Inconsistencies in bone mineral density testing\u00a0The\u00a0Better Bones,\u00a0Better\u00a0Body\u00a0perspective on PEMF\u00a0therapy\u00a0While there is a considerable body of research looking at the effects of various PEMF\u00a0fields\u00a0on\u00a0test-tube\u00a0bone cells and\u00a0animal\u00a0bone health,\u00a0only a few studies have\u00a0been conducted\u00a0with\u00a0living\u00a0humans. And these\u00a0yield\u00a0inconsistent results.\u00a0Every cell in the body responds to electromagnetic forces and\u00a0to\u00a0the flow of electrons,\u00a0and I expect that\u00a0one\u00a0day\u00a0scientists will identify the specific waveforms,\u00a0frequencies,\u00a0and duration of treatment\u00a0that\u00a0provide\u00a0a lasting effect on bone regeneration.\u00a0Keep in mind,\u00a0however,\u00a0that there is no single \u201cmagic bullet\u201d\u00a0that will resolve the osteoporosis epidemic.
